Before completing this form, we recommend that you read our How to Raise a Dispute guide and use the Claim Wizard to ensure that you are directing your complaint to right people. We would also urge all consumers to ensure that the gambling operator they are in dispute with is aware that you are planning to seek the assistance of IBAS.

Please ensure that you have read our data policy and service terms. In order to investigate your complaint we will need to ask your nominated gambling operator questions about your account and betting or gaming activity.

We do not guarantee that we will able to consider every request for adjudication. If we are unable to consider or complete the adjudication process we will inform you either by e-mail or post.

We also kindly request that you advise us if the dispute is resolved with the gambling operator after you have completed this form.
